The following is a list of printable Acrobat PDF documents used to apply for various ministry positions at Grace. Some documents may require a password to open, so you must contact the church office for that information (for legal purposes).

Spiritual Gifts Assesment

This analysis seeks to highlight your spiritual preferences and tendencies. Please print and complete the assessment and return it to Pastor Bryan Brotherton for analysis.

Christian Service Inventory

This lists a wide range of ministries here at Grace and in the community and asks you to identify your past and present involvements, as well as indicate areas you might have interest in participating in the future. Please download, print and complete the assessment and return it to Pastor Bryan Brotherton for analysis.

Youth Child Protection Program Form

This must be filled out by any youth age 14-17 planning on serving with any ministry that works with minors at Grace. This PDF download requires you to have a password to open the PDF Document once it is downloaded (for legal purposes). Please call the church office to obtain the password to open this document.

Adult Child Protection Program Form

This must be filled out by anyone 18 or older planning on serving with any ministry that works with minors at Grace. This PDF download requires you to have a password to open the PDF Document once it is downloaded (for legal purposes). Please call the church office to obtain the password to open this document.

Youth Medical Release Form

This PDF download must be filled out and given to the church office or youth staff if your child plans on attending a trip or event with Impact (Senior High) or 180 (Junior High) groups. Must be completed and turned in or on file for your child to be able to attend trip/event.

Awana Registration Form

This PDF download must be filled out and given to the Awana Secretary.
